EMT Mundell killed in ambulance, 18 wheeler crash - Coolidge, Arizona

Tammy Mundell, 29, an EMT for the privately owned Southwest Ambulance company, died from injuries sustained in an ambulance crash.

The collision occurred at the intersection of Highways 87 and 287 near the town of Coolidge.

The ambulance was struck by an 18-wheeler gravel truck and also involved three other vehicles. The entire front of the Southwest Ambulance was sheared off in the accident.

Mundell and Paramedic Sandra Williams were transporting an inmate and a detention officer from the Arizona State Prison Complex-Florence. Mundell was driving the ambulance when it collided with the truck. Three others were riding in the ambulance including a prisoner, a guard and a paramedic.

Southwest Ambulance employee, Paramedic Sandra Williams, is in critical condition at Maricopa Medical Center in Phoenix from the crash.

A prison inmate and corrections officer from the state prison in Florence also in the ambulance were flown to Valley hospitals. Their conditions were not available.

In all, six people were injured. They were transported to Maricopa Medical Center.

The investigation continues.
